SMS Messaging Service

By providing my phone number, I consent to receive SMS text messages from LEAG Management NJ LLC, DBA, Linea Wellness and Beauty Lounge for appointment reminders, marketing messages, and general two-way communication about aesthetics and wellness services. Message and Data Rates may apply.

Message Frequency

You will get more than one message from us unless you opt-out, and while messaging frequency varies, you will likely receive 4 messages per month. LEAG Management NJ LLC, DBA, Linea Wellness and Beauty Lounge reserves the right to alter the frequency of messages at any time to increase or decrease the total number of messages. LEAG Management NJ LLC, DBA, Linea Wellness and Beauty Lounge and carriers are not liable for delays or undelivered messages.

Cancellation/Opt-Out Instructions

You can opt out of receiving SMS messages at any time by replying STOP to any message we send you. After you opt out of text messaging, you will receive one additional message confirming your request has been processed.

We are not responsible for any charges, errors, or delays in SMS delivery or undelivered messages caused by your carrier or third-party service providers.

Terms of Service Regarding HIPAA

Patient Privacy and Confidentiality: We are committed to protecting the privacy and confidentiality of your Protected Health Information (PHI) as required by the Health Insurance Portability and Accountability Act (HIPAA) and other applicable laws. 

Notice of Privacy Practices: We will provide you with a Notice of Privacy Practices (NPP) that explains how we may use and disclose your PHI, your rights regarding your health information, and how to contact us with questions or concerns. 

Use and Disclosure of PHI: We will only use or disclose your PHI for treatment, payment, and healthcare operations, or as otherwise permitted or required by law, or with your written authorization. 

Security of PHI: We will implement reasonable administrative, technical, and physical safeguards to protect the security and confidentiality of your PHI, including encryption and access controls. 

Patient Rights: You have the right to:

  • Access your PHI. 

  • Request an amendment to your PHI. 

  • Request an accounting of disclosures of your PHI. 

  • File a complaint with us or with the Department of Health and Human Services (HHS) if you believe your privacy rights have been violated. 

Business Associate Agreements: We will enter into Business Associate Agreements (BAAs) with any business associates (e.g., vendors, contractors) who have access to your PHI, ensuring that they are also obligated to protect your privacy and security. 

Breach Notification: We will promptly notify you and, if required, the HHS, of any breach of your unsecured PHI. 

Compliance: We are committed to complying with all applicable HIPAA regulations and will take appropriate corrective action if any violations are identified.
